Electronic Health Record (EHR)
MES provides a secure EHR repository to store faxes, documents, medical images, and more. Paper-based originals can be scanned and stored online virtually eliminating paper. The EHR provides a lifetime record of your clients’ encounters with your agency and allows consent-based access to the EHR from within the office and at the point of care. Referral and Intake – You need efficient processes to support intake and admission tasks. Fax capture and document management use familiar windows drag and drop processes. Assigning health care worker access is point and click. In agency benchmarking, processing incoming referrals was reduced from 10 minutes to 2 minutes and completely eliminated paper.
Assessment – MedShare’s design allows us to integrate any assessment tool within the EHR. Our patent-pending Smart Forms technology is used within MES to create assessments that have embedded guidance, edit and audit control and allow initial and re-assessments to appear on the same forms. The underlying data is stored in XML format allowing integration with other forms, administrative systems and transmission to others within the care team.
Care Planning - Care Plans are a powerful decision support tool that enable the development of a personalized care plan for each client’s unique needs. Care Planning templates developed by the agency incorporate clinical best practices for starting client specific care plans. The data from care plans can be mined to support continuous quality improvement and support outcome-based medicine.
eCharting - Smart Charts™ begin as a replacement for paper-based charting, but offer much more. Clinical guidance is embedded into the chart to provide education leading to continuity of information amongst care givers. Smart Charts allow multiple observations over time providing the ability to see changes and trends in the client’s condition leading to improved care.
Progress Notes - MES Progress Notes are stored centrally and shared amongst members of the care team.
